• Developing a tailored approach through close collaboration with individuals, including those with neurodivergent or other physical and mental challenges
• Acitvely listening to aspirations, preferences, and needs and customised nutrition support plans and services to align with individuals unique goals.
• Executing a multifaceted role encompassing comprehensive needs assessment, strategic nutrition support plan development, seamless service coordination, direct liaison with service providers, and facilitating participants' access to tailored and suitable dietary services.
• Cultivating effective collaboration with a diverse range of stakeholders, including participants, families, service providers, and healthcare professionals, by employment of comprehensive and holistic including dietary solutions and other vidence-based therapies (e.g. CBT, FBT, and ACT).
• Engaging with participants and their families with compassion and empathy, fostering trust and rapport while sensitively addressing their challenges and aspirations.
• Enhancing communication by translating complex nutritional concepts into accessible language or utilising multilingual resources, creating a supportive environment.
• Adapting nutrition support plans in response to changes in participants' goals, nutritional requirements, and health conditions, ensuring optimal outcomes throughout their journey.
• Maintaining and updated records of participant interactions, assessments, support plans, and progress reports with a high level of accuracy, adhering to NDIS documentation guidelines and compliance standards.
• Ensuring compliance with federal and local regulations, including DA, NDIS, DVA, and ANZAED guidelines, in all dietary interventions, guaranteeing services of the highest quality and safety standards for participants.
• Fostering a culture of continuous improvement and innovation in dietary interventions, striving to enhance participant outcomes and overall program effectiveness.
• Delivering services with professionalism, empathy, equality, cultural awareness, flexibility, and compassion.
• Continuing professional development with available evidenced based resources or engaging in accredited courses/webinars/symposiums, plus engaging mentors.
• Developed resources resulting in a 15% reduction in participants' average cholesterol levels within three months, significantly improving their cardiovascular health and overall well-being
