About the research trial
The ACI Transition Care Network would like to invite young people with chronic conditions, their families and carers to take part in a survey to share their experiences of moving from children’s to adult health services in NSW. This information will help the ACI Transition Care Network to form our future plans and priorities and provide opportunities to work more closely with consumers on improving transition processes for young people with a chronic condition in NSW.
